Sample Diagnosis
INITIAL DIAGNOSIS
1. The patient sends a valid sample for testing.
2. Enclose a short descriptions of your problems (signs & symptoms).
3. List or all medications you are currently taking.
4. Send a self-addressed Courier-Easy Track with:

A VALID SAMPLE should be one of the following:
1. A few drops of urine on a piece of white tissue paper.
Or
2. A spit sample on white paper tissue (do this first thing in the morning
before you eat, drink, and clean your teeth).
Dry the sample, and then wrap the sample in glad wrap to prevent contamination.
The sample must be dry before sending. The sample will not be tested when
wet.
NOTE: WHEN YOU RECEIVE COLOURS
Loan/Rental Agreement:
The colours/machine received are on loan/rent. They are due on the due date. If the colours/machine are received later than the due date there will be accumulating charges of $40.00/$50.00 per week, either /or colours missing or lost will be charged at the rate of $100.00 per colour after accumulating charges till notification of loss. Machine (parts) damaged on return, client is liable for repair or replacement.
Ownership of goods remains the property of ACE Colour Therapy LTD until paid in full. Payments of goods/service on date of invoice or on deliver of goods/service. Client will be liable for collection fees and administration fees. Client is liable for collection fees should cheque be dishonored.
